PAYMENT MUST ACCOMPANY THE APPLICATION TO BE CONSIDERED FOR MEMBERSHIP

1.            There is a $100.00 nonrefundable application fee. The annual dues are $180.00 and nonrefundable.

2             Membership is with the company, not the individual. The company names the primary member.

3.            Secondary memberships are $35.00 each; there is no limit on the number of secondary members.

4.             Dues are prorated quarterly:  $180.00, $135.00, $90.00 and $45.00.

5.             Cash, check, Master Card or Visa are accepted.
